To request warranty, please complete the following request for information. If you require a replacement part, a purchase order is required. At our discretion the replacement part shipped to you may be a reworked component. Once the form is completed, an RMA# will be issued for the return of the defective part. The defective part will have to be returned for evaluation. You will be notified of the evaluation and subsequent actions. (All fields must be filled out before Submit button sends.)

Note 1:
Serial number is located on the top of the slide as per the photos above.

Note 2:
There is to be no welding until it is approved by an authorized representative of The Curotto Can. Unauthorized work will void warranty.

Note 3:
A purchase order # is required in all cases. When the defective part is returned and evaluated, you will be credited for the part if it is determined it is indeed covered by warranty.
